How it works in practice
Every page is undated on purpose, so you can start it at any point in a diagnostic process that may already be years old — approximate months are far more useful than no dates at all. It asks you to write things down once, on the day, while the wording is still fresh; it does not ask you to track anything daily or to keep it up when you are too unwell to. The last two pages are written to be taken with you and read out.
