Chronic Illness Flare Tracker

A focused log for the days your symptoms spike — trigger, severity, duration and what helped — so the pattern behind your flares finally becomes visible.
Flares can feel like they come out of nowhere, until you start writing them down. This 6-page printable tracker exists to catch the details a flare usually erases from memory once it's passed — what came before it, how bad it got, how long it lasted, and what actually got you through it.

Inside the planner

How it works in practice
Every page is undated on purpose, so you can start logging the moment a flare hits, on any day, in any month. A few months of entries usually says more about your real triggers than memory alone ever could — and the monthly summary turns "I've just been having a rough patch" into something specific you can bring to an appointment.
