Chronic Illness Wellbeing Journal

A gentle weekly pause for the side of chronic illness the symptom logs don't cover — what helped, what you need more of, and one kind thing you'll do for yourself.
Managing a chronic condition is only half tracking symptoms. The other half is making sure you're not disappearing into the management of it — that somewhere between the medications, the appointments and the pacing, there's still room to notice how you're actually doing. This 6-page printable check-in is that room. It takes a few minutes, once a week, and it isn't about fixing anything. It's about pausing long enough to notice what helped, what you need more of, and what you managed, even on the hard weeks.

Inside the planner

How it works in practice
Every page is undated on purpose, so you can start on any week of any month and pick whatever day works for your routine. Each weekly page asks the same few honest questions — what helped, what you need more of, what you managed well and who supported you — so answering becomes a habit rather than a chore. After three weeks, the Monthly Reflection page helps you look back across all of them and notice the pattern, and the boundary or change that would help most going forward.
